This page covers onboarding, site rules, working-at-height training, toolbox talks and site acronyms.

Overview

The current working-at-height procedure applies to climbing, nacelle and hub work, and rescue across the site, and its purpose is to prevent falls of people and objects and ensure quick casualty rescue.12 The current induction rules require sign-in and sign-out, a permit to work for every turbine job, no lone working for turbine access, required personal protective equipment outside vehicles, and same-day reporting of incidents and near misses.34 Training arrangements include working-at-height and rescue refresher training every two years, rescue drills twice a year on site, and toolbox talks for specific lessons such as dropped objects and cold-weather start-up after the oil conversion.567 The site uses acronyms including LSS for low-speed shaft, IMS for intermediate-speed shaft, HSS for high-speed shaft, PQ index for particle quantifier index, and RMS for root mean square.8

Working at height rules

  • The O&M Supervisor plans work at height and checks weather limits, while each technician must check his or her harness and lanyards before use.910
  • Tool tethering is mandatory above 2 m, and hatches must stay closed while tools or parts are passed.11
  • During nacelle work, an exclusion zone with signage is set up at the tower base, and nobody enters it without the nacelle team's agreement.11
  • Climbing requires at least two persons, with only one ladder section climbed at a time, and climbers must use the fall arrest rail and rest platforms and use the service lift when available.1213
  • Damaged ladder rungs or fall arrest components must be reported immediately.14

Weather and access limits

  • Climbing and nacelle work are limited to a 10-minute mean wind speed up to 15 m/s, hub entry is limited to a 10-minute mean wind speed up to 12 m/s, and nacelle roof work is limited to a 10-minute mean wind speed up to 10 m/s.151617
  • Climbing is not allowed during a thunderstorm when lightning is detected within 15 km.18
  • For ice, there is no access within 300 m of a turbine with ice detected under alarm A7001.19
  • The induction states that ice detection alarm A7001 stops the turbine automatically and that personnel must keep 300 m away from a turbine with ice on the blades.20

Rescue readiness

  • Each nacelle has a sealed rescue kit with a descent device.21
  • Rescue kits are inspected every 6 months, and the seal is checked on the daily checklist.21
  • A broken rescue kit seal means the kit must be inspected before the next climb.21
  • The rescue procedure begins by raising the alarm to the control room and emergency services, then making the turbine safe by stopping it and applying the rotor lock, and then attaching the casualty to the descent device and lowering the casualty to the ground or through the tower.222324
  • The rescue target is to have the casualty on the ground within 15 minutes of the alarm.25
  • The HSE Coordinator organises rescue drills twice a year.26

Dropped-object lessons

  • Near miss INC-2026-003 involved a 24 mm impact socket weighing 0.4 kg that slipped from a technician's hand at the yaw deck while tools were being passed through an open hatch.6
  • The socket fell about 85 m inside the tower to the tower base platform; nobody was at the tower base because the exclusion zone was respected, there was no injury, and a cable tray cover was dented.6
  • The identified causes were that the socket was not tethered, the hatch stayed open while tools were passed, and there was no tool tether anchor point at the yaw deck hatch.6
  • The lesson learned was that exclusion zones under work at height are effective, and that tethering every tool and keeping hatches closed removes the hazard at its source.6
  • SOP-WAH-003 Revision B added mandatory tool tethering above 2 m after near miss INC-2026-003 and added the requirement to close hatches when passing tools.27

Induction and site rules

  • The site has 12 Litware LT-32 turbines in CCV configuration, identified WTG-01 to WTG-12, and the O&M building with workshop and stores is at the site entrance.28
  • The speed limit on all site roads is 20 km/h.3
  • Isolation follows SOP-LOTO-001 and requires a personal padlock and name tag.4
  • High-voltage switching is only for authorised persons, and the control room must always know who is in which turbine.29
  • All work at height must follow SOP-WAH-003.30
  • In an emergency, the alarm is raised to the control room first, the muster point is the O&M building car park, and the site ERP is followed.31
  • Automatic turbine stops for permit reasons must never be overridden.32

Training, talks and records

  • The internal safety audit scope included training records, working at height and rescue, PTW, lockout/tagout, lifting equipment, oil and chemical handling, and emergency arrangements.33
  • The audit found incomplete toolbox talk attendance records for November 2025, with one of four sessions missing a sign-in sheet, and set an action to reconstruct the records and use a digital sign-in.33
  • The dropped-object near miss led to a dropped-objects toolbox talk on 2026-03-26.6
  • The gearbox oil management-of-change action list included a toolbox talk on cold-weather start-up with the new oil.34
  • The cold-weather start-up toolbox talk asked attendees to sign the attendance sheet before leaving.35

Gearbox oil training points

  • The management-of-change record for the gearbox oil grade change replaces Proseware PWG 320 with Proseware PWG 220 in the gearboxes of all 12 turbines.36
  • For the oil-mixing hazard, the identified risk is residual PWG 320 above 5 %, controlled by a 60 L flush, labelling the filler, and removing PWG 320 drums from the shelf.37
  • For the cold-start hazard, the control is to set GBX_TMIN_START to 5 °C and verify it in the conversion checklist.38
  • The training deck states that, since SB-LWS-2025-07, the gearboxes use Proseware PWG 220 with ISO VG 220 instead of PWG 320, and that oils must not be mixed, with top-up using PWG 220 only.39
  • The cold-weather toolbox talk states that, after conversion, a gearbox must never be topped up with PWG 320 and that old PWG 320 drums are marked do not use.40

Acronyms and technical terms

  • LSS means low-speed shaft, the main shaft on the rotor side.8
  • IMS means intermediate-speed shaft, located between the two helical stages.8
  • HSS means high-speed shaft, the output shaft to the generator coupling.8
  • PQ index means particle quantifier index, the amount of ferrous debris in an oil sample.8
  • RMS means root mean square, a value used for vibration velocity.8
  • The quiz answers state that the HSS drives the generator, A2102 trips at 82 °C, the oil used is Proseware PWG 220, and a rising PQ index means more ferrous wear debris, often from a bearing.41
